Stage 1: Engagement


After receiving the referral request, our administrative staff will meet with you to review your NDIS plan and determine how we can best assist you in achieving your NDIS objectives.

We may ask you to disclose information about yourself (such as your name, NDIS number, disability, and the most important persons in your life) so that we can understand your desired outcomes. Then, we will create a service agreement outlining how you and our Living Strength OT team will collaborate. If you choose us as your support coordination provider, you will be required to sign a service agreement, after which our administrative staff will match you with the support coordinator most suited to your requirements, objectives, and ambitions.

Stage 2: Planning


Your coordinator of matched assistance will contact you to schedule a planning meeting. We invite your support network to attend this meeting so we can completely comprehend your situation.

During the meeting, we'll discuss:

Your NDIS targets
Your supported funding categories
How Assistance Coordination Operates
Together, we will build an individualized, whole-life curriculum that matches your needs, objectives, and ambitions. This will assist you in achieving your NDIS plan's objectives.

Following the consultation, we will match you with the support workers and services of your choice.

Stage 3: Implementation


Your support coordinator will meet with you to discuss the accomplishment of your objectives. In addition, they will assist you in navigating the NDIS by linking and coordinating informal, mainstream, and sponsored services in a context of service delivery. We pledge to accompany you on your NDIS journey and help make it a reality.

Stage Four: Monitoring


Once your supports and services are in place, we will monitor your objectives, needs, and ambitions in collaboration with you. We will keep track of how your objectives are being met, which will provide you with vital information when the NDIA reviews your plan. This often occurs once a year.

Managing your own plan


As we work with you, we will also improve and enhance your abilities and self-assurance to manage your NDIS plan. Once you feel comfortable coordinating your NDIS plan, you may opt to independently manage your plan.

Over time, assume some of your case coordinator's duties and seek the resources you need to administer your own plan autonomously.
